Physical Exam Information

  • All returning and new student-athletes will need to enter their medical history, insurance information, and sign waivers via our online data entry system (SportsWare). To enter your information, visit www.swol123.net. The first time you visit the website you will need to click "Join Sportsware".   The school ID is: VU Lions
    • Fill out your first and last name, email address and click Vanguard for the "group" then send. Once you have been approved in the system (approximately 24hrs) log back into Sportsware and fill out your information in each tab.  Upload a front and back copy of your primary medical insurance.  Fill out the forms in "attachments" tab. We also ask that you please print and fill out the NEW Health history questionnaire forms and mail to: Athletic Training 55 Fair Dr. Costa Mesa, CA 92626 , or bring the day of your physical. 
 
  • For returning VU student-athletes, physical fee for all VU student-athletes is $25.00. Please bring in the form of cash or check (Checks payable to Vanguard Athletics) or you can follow this link and pay online HERE.

     
  • Your athletic PPE must be conducted by the Vanguard University Sports Medicine Staff on the specified dates during the Spring semester. Our physicians will have the final decision on your sport participation regarding your athletic health.
 
  • If you are not from Orange County and will be participating in Vanguard Athletics, it is required that you find a Primary Care Physician in this area that is a provider for your insurance plan. We recommend finding a physician that is part of the IPA Greater Newport Physicians Group if your insurance allows. This is a mandatory policy because if you were to wait until you were hurt to find a Primary Care Physician in Costa Mesa, you would not only decrease your immediate treatment options, but you would also miss out on valuable sport participation time. 
 
  • The VU Sports Medicine Staff is making it mandatory to provide proper documentation regarding every student athlete's status on whether or not they carry the Sickle Cell Trait. An information packet and letter is available online HERE to all student-athletes and their parents. This packet has the steps every student-athlete MUST take PRIOR to PPE's to provide proof of Sickle Cell Trait Testing, as well as educational material on what the Sickle Cell Trait is and how it can affect student-athletes. Please complete this as soon as possible as it may take SEVERAL weeks to get the information.
 
  • If you are going to be under the age of 18 on the day of your PPE, you must have a parent or legal guardian with you.
 
  • We will not accept PPE's you have received for other reasons (i.e. employment, previous schools, etc.).
